A condition to hire KEYTLaw, LLC, to be your statutory agent is you must read and accept our Statutory Agent Service Agreement. Check this box to confirm that the contact person completed and submitted this Questionnaire and he or she confirms that he or she has read the Statutory Agent Service Agreement and accepts it as the authorized agent on behalf of the entity or person.
When you pay us to be your LLC's statutory agent we will immediately send you an email message that asks you to confirm your email address before we will give you the signed Statutory Agent Acceptance form. You must file this form with the Arizona Corporation Commission along with another form called "LLC Statement of Change of Known Place of Business Address or Statutory Agent." Click on the link in the email and you will immediately get a second email message that contains a link to the Statutory Agent Acceptance form signed by Richard Keyt on behalf of KEYTLaw, LLC.
Click on the link in the second email then download the Statutory Agent Acceptance form to your computer. Make a note of the folder where you save the form. The file name is acceptance.pdf. Close the page on your browser that displays the acceptance form. Open the Statutory Agent Acceptance form on your computer. In question 1 "Entity Name" type the exact name of your company. In question 3 "Statutory Agent Signature" to the right of Richard Keyt, Manager correct the date to the date you print the form. Save the pdf file. Print the finished form.
After you hire us it is your responsibility to file the appropriate paper documents with the Arizona Corporation Commission. You must: (i) if you are appointing us as the statutory agent for an existing company prepare the ACC's LLC Statement of Change of Known Place of Business Address or Statutory Agent form or if you are forming a new company, prepare the ACC's Articles of Organization, (ii) arrange for a member of a member managed LLC or a manager of a manager managed LLC to sign this form, (iii) file the form and the Acceptance of Statutory Agent form we give you with the Arizona Corporation Commission, and (iv) pay the filing fee. The filing fees for the LLC Statement of Change of Known Place of Business Address or Statutory Agent form are $5 (regular review period) and $40 (expedited review period). The filing fees for the Articles of Organization form are $50 (regular review period) and $85 (expedited review period).
