In the field below indicate the name(s) of all people, entities and/or trusts who you want to inherit your LLC interests and the percentage to go to each named heir. Indicate the relationship of people to you. If you do not indicate the percentage to go to each heir the Transfer of Membership Interest Testament will go equally to all heirs named below.

For example, if you want 60% to go to Bart Simpson and 40% to go to Lisa Simpson you would enter Bart Simpson, nephew, 60% and Lisa Simpson, niece 40%.
