Domain Name Cease & Desist Letter Questionnaire

This Cease & Desist Questionnaire is to be used only by trademark and service mark owners (registered and unregistered) who want to hire KEYTLaw, L.L.C., to prepare and send a domain name cease & desist letter to one or more domain name registrants who the mark owner believes are infringing on the mark owner's mark(s).

Instructions

1. Complete all blank spaces in the Questionnaire.  If necessary attach additional pages of information.

2. Compile the trademark/service mark information requested in Part 3 and send it with the Questionnaire to KEYTLaw.

3. Have the mark owner or the mark owner's authorized representative sign and date the Questionnaire.

4. Prepare a check payable to KEYTLaw, L.L.C. in the amount of $495 plus $15 for mailing costs for each letter to be sent.  A domain name cease & desist letter will be sent to only a single domain name registrant for the fixed fee of $495.

5. Mail or send the completed and signed Questionnaire, all supporting documentation and your check in the proper amount to KEYTLaw, LLC.

6. NOTE:  If you do all of the preceding 5 steps, you will not become a client of KEYTLaw, LLC, unless and until: (i) KEYTLaw first determines that it would not have a conflict of interest if it represents you in connection with the proposed letter(s), (ii) the Questionnaire has been properly signed by the mark owner, (iii) all required advance payments have been received, and (iv) KEYTLaw accepts the representation by signing the Questionnaire and mailing or faxing a copy of it to the contact person for the mark owner as listed in the Questionnaire.
