Enter the exact name of the nonprofit corporation. It is optional to include the term Inc., Incorporated, Corp. or Corporation after the name. However we do recommend one of those terms so that people who deal with the corporation cannot claim they were unaware that they were dealing with a corporation.

If you select this option we will work with the directors to determine criteria for membership and modify the Bylaws accordingly. If our time to modify the Bylaws for your membership criteria exceeds 1 hour will bill the company $250/hour for the time in excess of one hour.

Address of the Corporation's Principal Place of Business in Arizona*
Arizona law requires the nonprofit corporation have an address in Arizona. The address can be a home address or a business address, but it cannot be a U.S. Post Office box. If the corporation does not have an Arizona address or does not want the address to be on the public records of the Arizona Corporation Commision, check the box that follows this field to purchase KEYTLaw, LLC's address service for $100/year.
