Arizona Estate Planning Forms

Richard Keyt prepares basic and complex estate plans for Arizona residents.  For many people, a basic estate plan is sufficient.  A basic estate plan consists of a Will, Durable Health Care Power of Attorney, Living Will, Prehospital Medical Care Directive, Durable Mental Health Care Power of Attorney, Durable General Power of Attorney, and a Letter to Your Representative. 

Complex Estate Plans

In general, married couples who have taxable estates over $4,000,000 in 2006 or who have a need for a trust need a complex estate plan.  Complex estate plans may avoid probate, substantially reduce federal estate taxes and provide for the long-term management and disposition of assets after death.  Richard Keyt also prepares complex estate plans.

Arizona Estate Planning Forms

Rick Keyt prepares custom versions of all of the following estate planning forms as part of each estate plan.  However, for those who prefer to do it themselves rather than use the services of an attorney, the Arizona Attorney General created the following documents for use by adult Arizona residents:

Durable Health Care Power of Attorney - Use this form to designate a person and an alternate person to make decisions about your medical care if you are unable to make those decisions.

Living Will - Use this form to notify your health care provider(s) that if you are in a terminal condition, a persistent vegetative state or an irreversible coma, you do not want your life to be prolonged by life sustaining treatment, beyond comfort care, that would serve only to artificially delay the moment of death.

Prehospital Medical Care Directive (Do Not Resuscitate) - Use this form to notify your doctor and emergency medical technicians or hospital emergency personnel not to resuscitate you if you suffer cardiac or respiratory arrest.

Durable Mental Health Care Power of Attorney - Use this form if you want to appoint a person to make mental health care decisions for you if you become incapable of making those decisions for yourself.

Letter to Your Agent (Representative) - Use this form to notify your agents/representatives that you have named them in one or more of your estate planning documents and they have responsibilities thereunder.

Life Care Planning Packet - All of the above forms plus additional information provided by the Arizona Attorney General.

The above forms are in .pdf format, which requires the Adobe® Acrobat® Reader® software.  You can download the software for free from Adobe®.
