The following is a partial list of Arizona real estate related forms prepared by the Arizona Association of Realtors^® for use by Arizona realtors and the general public. Residential Rental Forms

	Residential Rental Agreement
	Lead-based Paint Disclosure - Rental - landlords of homes built before 1978 MUST complete this form, deliver it to the tenant(s) and have the tenant(s) sign it

Purchases & Sales Forms - Homes

	Residential Resale Purchase Contract - fill in the blanks type form used by most Arizona realtors - includes seller disclosure requirements
	Addendum - if you want to make changes or additions to the Residential Resale Purchase Contract use this Addendum and attach it to the RRPC.
	Counter Offer - use this form to make a counter offer
	Assumption / Carryback Addendum - use this form if the seller will loan money to the buyer(s)
	Lead-based Paint Disclosure - sellers of homes built before 1978 MUST complete this form, deliver it to the buyer(s) and have the buyer(s) sign it
	Residential Seller's Property Disclosure Statement (SPDS) - sellers of homes MUST complete this form, deliver it to the buyer(s) and have the buyer(s) sign it
	Domestic Water Well/Water Use Addendum - Seller's Property Disclosure Statement - if the property has a well, the seller MUST complete this form, deliver it to the buyer(s) and have the buyer(s) sign it
	Buyer Inspection Notice with Seller's Response - to be used by a buyer to notify the seller of the results of a home inspection. Sellers may also ask the buyer(s) to sign it if the buyer(s) completed an inspection or elect not to perform an inspection
	Conditional Loan Approval - to be completed by the buyer(s) prospective lender

Purchases & Sales Forms - Vacant Land

	Vacant Land Purchase Contract
	Addendum to Vacant Land - if you want to make changes or additions to the Vacant Land Purchase Contract use this Addendum and attach it to the VLPC.
	Counter Offer - use this form to make a counter offer
	Assumption / Carryback Addendum- use this form if the seller will loan money to the buyer(s)
	Vacant Land/Lot Seller's Property Disclosure Statement (SPDS) - sellers of vacant land MUST complete this form, deliver it to the buyer(s) and have the buyer(s) sign it
